Getting Started at Ample Grace Psychiatry

We are glad you are here. Whether this is your first time seeking psychiatric care, you are transferring from another provider, or you are returning to care after a gap — getting started is straightforward. Here is exactly what to expect.

Three Steps To Your First Appointment

Step 1

Book your appointment online

Choose a new patient appointment through our secure online scheduler at amplegracepsych.com. No referral needed. Most new patients find availability within 5–7 business days. If you have a specific scheduling need, call us at 800-223-4914.

Step 2

Complete your intake packet before your visit

After booking you will receive a secure email with a link to complete your pre-appointment intake forms. This includes consent forms, a brief medical history, current medications, and screening questionnaires. Most patients complete this in under 15 minutes. Please complete your intake at least 24 hours before your appointment so your provider has time to review it.

Step 3

Join your telehealth visit

Thirty minutes before your appointment you will receive a secure link to join your video visit. You will need a device with a camera and microphone and a stable internet connection. Be in a private, quiet location where you can speak freely. Your first appointment is 45–60 minutes.

What To Prepare

Before your first appointment, have these ready

  • A list of all current medications, supplements, and vitamins including name, dose, and frequency
  • Names and dates of any previous psychiatric diagnoses or evaluations
  • Names of any previous psychiatric medications and the reason you stopped taking them
  • Your insurance card and photo ID
  • A private, quiet location with a working camera and internet connection
  • Any questions you want to make sure you ask — write them down so you do not forget
For Transferring Patients

Transferring from Another Provider

We actively welcome patients who are transferring from another provider. Your intake forms include sections for your complete psychiatric history, current medications, previous diagnoses, and your preferred pharmacy. If you would like to have records sent from your previous provider, we can provide a release of information form.

If you have an active prescription that needs to be continued, please note this in your intake and bring your current medication bottles to your first appointment. We will review your medications and discuss your care plan at your initial consultation.

Cancellation Policy

Cancellation and Rescheduling

We ask for at least 24 hours notice if you need to cancel or reschedule your appointment. To cancel or reschedule, call 800-223-4914, send a portal message, or email info@amplegracepsych.com. We understand that emergencies happen — please reach out as soon as possible if you are unable to attend.
